I am the legal guardian of the child listed below and do hereby consent for the below mentioned student to travel with First Baptist Church of Crawford, Texas, to Giddings, Texas, to participate in Preteen Camp at Camp Tejas from July 17, 2026 to July 20, 2026.  Additionally, I grant permission for representatives of First Baptist Church of Crawford, Texas or other camp staff to obtain necessary medical attention for the aforementioned student in the event of illness or injury, either of which are highly possible in a camp setting. I also recognize that misconduct by my student may result in the discontinuation of my student’s time at camp, and I will be responsible for travel arrangements and cost if my student is sent home. I also acknowledge that pictures/videos of my student may be used to promote camp or FBC Crawford on church or camp social media unless I deny their use in writing to First Baptist Crawford.

I, the undersigned, do hereby verify that the information on this form is correct, and I do hereby release and forever discharge First Baptist Church of Crawford, Texas, and its representatives or sponsors from any and all claims, demands, actions or cause of action past, present or future out of any damage or injury while traveling to/from and participating in this event.
