By completing the following form I consent to the following:
I am the legal guardian of the child listed below and do hereby consent for the below-mentioned student to travel with First Baptist Church of Crawford, Texas, to Waco Surf (Address: 5347 Old Mexia Rd, Waco, TX 76705), and back to the church, on Friday, July 26, 2024. Additionally, I grant permission for representatives of First Baptist Church of Crawford, Texas, or Waco Surf staff to obtain necessary medical attention for the aforementioned student in the event of illness or injury, either of which is highly possible in a Water Park setting. I also recognize that misconduct by my student may result in the discontinuation of my student’s time at Waco Surf, and I will be responsible for travel arrangements and costs if my student is sent home. I also acknowledge that pictures/videos of my student may be used to promote FBC Crawford on church social media unless I deny their use in writing to First Baptist Crawford.
I, the undersigned, do hereby verify that the information on this form is correct, and I do hereby release and forever discharge First Baptist Church of Crawford, Texas, and its representatives or leaders from any and all claims, demands, actions, or cause of action past, present or future out of any damage or injury while traveling to/from and participating in this event.

If your student is 16 and under the cost of the pass is $12. If your student is older than 16 the cost of the pass is $25. If you're a leader who has been invited by Noel, there is no cost for your pass.
In addition, food will not be provided during this time, so please give your students money for snacks and or merch if they would like something.
